What types of insurance do we accept?
We are in network with Blue Cross Blue Shield, but will bill out of network for all other insurance plans. We are contracted with the Division of Developmental Disabilities (DDD) and will accept Empowerment Scholarship Account (ESA) payments. Private pay clients are also welcome.
Where is our clinic located?
- Our primary clinic is located in Chandler at McQueen and Warner Roads.
- We have an additional satellite location in Tempe at McClintock Drive and Guadalupe Road.
What do you need at our first session?
We will need to obtain all prior authorizations from insurance, Division of Developmental Disabilities, etc. In addition, we will need the New Client Information Form fully completed, with a copy of the front and back of your insurance card(s).
What happens at our first session?
At your first session, your therapist will review your child's medical history, current performance, and your desired goals.
